A New Free Service to Support You

El Centro de Amistad is proud to introduce Enhanced Care Management (ECM), a new, free service designed to help individuals better navigate their mental and physical health needs.

 

Taking care of your health can feel overwhelming, but you don’t have to do it alone. ECM provides personalized, compassionate support through a dedicated care team that works alongside you to coordinate services, manage appointments, and connect you to essential resources.

 

Whether you need help finding healthcare providers, arranging transportation, or accessing community resources like food and housing, our team is here every step of the way. ECM is especially beneficial for individuals experiencing unstable housing, frequent hospital visits, pregnancy or postpartum needs, or those managing mental health or substance use conditions.

 

Through this program, participants receive hands-on guidance tailored to their unique needs, making it easier to stay on track and focus on overall well-being.

 

Best of all, this new service is completely free.

Please note: ECM is currently available to Molina members only, with additional health plans expected soon.
